If you have a sound card, be sure the drivers are up to date. When I first tried to play LotRO I could not even get the game to run until I updated my sound card drivers.
City of heroes has a good collision system. A tank or two can defend at a doorway and keep a whole swarm of mobs from getting past. All I need now is a mmorpg that doesn't let us shoot through other people.
Yes, although I'm not sure how many people do it. My guess would be not many.
I've never seen a character that looks like a dwarf. Maybe you have your sliders pushed all the way to short and beefy?

So I just started playing Hellgate and I know it was lacking at launch, but I have a couple questions.
Are these things in the game: Mail, PvP, Auction House
And I'm assuming Flagship Studios failure means no expansion?
Mail: yes
PvP: yes
Auction House: no
There was an expansion of sorts, or at least a big patch on the test server when Flagship had its meltdown. It may be that an auction house was part of that expansion, I cant remember. If anyone decides to invest in the development of the game then this could be released fairly soon. But at the moment everything is tied up in legal issues and I suspect it will remain so for some time.
Hellgate London has a perma-death mode which I quite enjoy. Nobody uses it for pvp though. It's all about thoughtful, "realistic" pve. An unofficial forum poll suggests that something like 10 to 15% of the players go for the perma-death mode. Unfortunately it is only available to subscribers and no new subscriptions are being accepted.
I tried Dofus back when I first heard about their perma-death server. I can't think of a single nice thing to say about Dofus. At least it's free so you can try it and judge for yourself.
