Feats probably play a big part when it comes to hate management. I noticed most of the classes have feats that lower the amount of threat they and their pets generate, and the soldier classes have feats that increase the amount of hate they generate.
Probably nothing anyone at all cares about low- and mid-level, so I wouldn't judge hate management at those low levels.

My wife and I are having the same problem. On the several occasions where I have spent several seconds getting aggro, only one spell from her causes all mobs to aggro her, no matter what hate-moves I've pulled.
Also, there have been a couple of occasions where I aggro the mobs but they run straight past me to the healer-wife when she is simply standing and waiting without having cast anything.
Taunt appears to be about 95% broken. I can pull a mob off her using a hate-move only rarely - even if she isn't casting, only swinging her hammer.
