This is a Herald of Xotli, which gets the mage fire fatalities, the standard 2he fatalities and some unique ones so a fun class to play, especially now that they fixed the hair and naked bugs. Fire UI by Bombata.  It's still in beta, but he's done a really nice job on it.  He's the maker of Strange UI and one older one I can't remember. 

Lots of little features that just work way better than the default UI.

Normally I hide the UI for screenshots, but it's really hard to get it hidden and a screenshot taken at the perfect moment in the fatality.

 

At least for me.
