I've been playing alot of dark souls lately (i know it's not a mmo) and i love that feeling of difficulty and the rush of the game is amazing.
I'm looking for something like that in the mmo world if possible ^^
Awhile ago I would have suggested FFXI, but...its changed. They have a 30 day free trial I do so believe, if you want hardcore, I'd say you can still scrap some from it, just try not to Grounds of Valor burn your main jobs (its a party that starts at level 15, you sit there with 21 other people and kill low level enemies, spamming the same page in the Grounds Manual for XP) it ruins some of the challenge, although you'll still find the EXP losss upon death and a majority of the new battles as difficult.

how do i get my hands on that trial? I'm finding it quite difficult...

I would say Darkfall Full loot there is not levels,manual aiming and you can use everything magic / melee / archery as hybrid at the same time.With your clan you can siege enemy holdings and take them as your owns,big sea fights with massive ships / waterfighting its best MMO and hardest aswell what i have ever played http://www.youtube.com/watch?v=OBqaOokNEtc&feature=plcp check it out

Pretty much there is only Darkfall and EVE, that is if you actually want fun and engaging gameplay, although EVE and DF are vastly different. EVE's combat is much more about strategy whereas DF combat is a lot more hands-on (player)skill based. But games feature full loot FFA PVP of sorts, but Darkfall is more familiar and easier to get into for most people. Aside from full loot PVP, DF offers many other features such as most advanced mob AI, FPS-style combat, guild-owned holdings, sieging of said holdings so one guild take holdings/cities from another guild, naval/ship combat, water/underwater combat, mounts and mounted combat, etc. But be warned, it's a brutal unforgiving world and veterans will eat you for breakfast until you get good (and I'm not talking about leveling/grinding your character).
Not sure what type of playstyle you go for, but if you like Space Sim/city management games then check out Shores of Hazeron It's a FFA PVP space sandbox. You can fully customize your own ships, and I don't mean choose your loadout. I mean you build your ships from square one. If you want to play it on extreme diffivulty then go attack some weaker players for no reason. You will then get to meet the peacekeeper governments who will swiftly bomb you back to the stoneage.
