I don't know about you but I am getting bored of the same variations of enviorments in mmos. Its either space, fantasy, modern world, old world (non fantasy), steampunk, and cyberpunk. Or different combinations of two. I'm sure someone can think of some other(s) that I have missed, just as sure as I am that other game developers can be more creative.
When i think of one I want to see, I'll run it by ya, but my brain has flat lined while typing this. So let me know your creative desires.

People changing things that work nicely around just so they are different.
The core thing to focus in a game is creating a challenge and having a game with depth and variance. Thats what people dont give a damn about.
Some people say, there are only 32 plots. Yet, we have how many fictional works?
Perhaps it is not necessarily the setting, but what is done and takes place within that setting.
You are seeing uncompelling worlds becuase almsot all the devs are using speedtree or some other auto generator.They even naminator npc names quite often.
As far as being Fantasy or Steampunk or Sci-fi,there is of course only so many ideas,evdery single idea has been covered except modern ones.The only idea i can think of is modernized worlds that would have shopping malls,hockey rinks,baseball diamonds,race tracks,but then you are getting more into a sims game.
There are several table top games,card games,2d version games like Fallen Sword,EVERY single aspect has been covered,you just need to look harder.Once you start getting into FUN and Action like example Unreal or Quake or Serious Sam,you are no longer in a MMORPG genre.Then there is literally thousands of games outside the genre as well,so it has all been covered.
