If you are in Beta, click the icon that says "Guild" and it will explain how to start one.

Guild Banners come later on, and are not required to start a guild.

you need to be in a team with 6 unguilded players.

the team leader can then aproach the guild registar and create the guild. That can be done in Ascalon for instance the Empire Capital etc.
