Community Member of the Week Application

The EverQuest II official site has been updated with and application for players who wish to submit themselves for Community Member of the Week.
As many of you already know we highlight a community member every Monday on EQ2Players. For the past 5 months or so, the community members highlighted were chosen by invitation; now we are prepared to let anyone participate if they wish. If you are interested in participating and being selected as a Community Member of the Week, please answer the questionnaire below and email it to me at cdelzer@soe.sony.com with EQ2 Community Spotlight in the subject line.
You are also welcome to submit a tasteful real life picture and screen shot of your character is you would like to. Feel free to disregard any question in the questionnaire you do not wish to answer.
NOTE: Highlighted community members are randomly selected from all the available questionnaires I have each week, so you may not see yours immediately upon sending it in.
