It is great concern of many on this forum and indeed the offical DDO forums that haveing only instanced areas is going to have a detramentla effect on the game. Mainly by removing the Multi from the MMO part of the genre. While Turbine have stated there will be areas, being called danger zones and shared instances, where players can meet other groups and solo characters, the question many people are having to ask, is "Is this enough?"

While I'll openly admit I like the instances to have our own private online DnD session in, I do sill want to bump into lots of other players and groups while they are doing their 'thing' :) Currently other than the above, the city of Stormreach has only taverns a few streets with vendors and a few shops, which are shared. To me this isn't very immersive and not at all like the the DnD I imagined to would be. Picture this, you or even your party go on a Quest, which is say to kill a Hill Giant, it's a long quest taking many different turns this way and that before you reach your quarry. Now while you may argue that you may never enounter another living soul outside, you can just as easily say otherwise. The solution to this would be having certain parts of the quest instanced leaving the remaining open to all.

Who knows, perhaps they already have this, and since the stress test was so short the people that got to play only saw a fraction of the available quests and areas?
