Hard mode vs normal mode raid ALREADY works in MMOs. You just need to apply this concept to more parts of the game.

Says who? MMOs are already not cohesive worlds. Why does it have to be consistent for everyone? If it makes a better game, make it inconsistent.
Games like D3 and WoW feel more like arcadey time wasters than virtual worlds.
So? Many play MMOs like arcadey time wasters. Haven't you seen the ones park in a city and wait for their dungeons to pop?
A world cannot be scary if everyone can just turn the scary off.
And why do MMOs have to be scary? WOW is not .. the most successful one out there. DCUO is not. DDO is not ....
People in WoW complain when things are hard because its a casual game. People in core MMOs complain things are too easy because they like challenge and depth.
Just make an MMO for a specific audience and keep to it. Bam.
MMOs are expensive. Too small an audience means little content, no features .. or even no game. It is an efficient solution to re-use the SAME content for different difficulties. Heck, even SINGLE PLAYER games have difficulty settings.
When DAoC switched from focusing on hardcore PvP and PvE, it lost ALL of its audience and didn't gain a new one.
So the lesson is that they should have BOTH .. as option and don't switch away to a single focus.
