EVE has it (dat carbon engine), Aion has it, and the new games that are coming have one iteration of character editors. even out of the mmo scene there's people making vehicle editors and we had building editors in games like The Sims.
i've been looking for the theory in the internet but i just seem to find Unity tutorials, right now i have designed how it will be the interface of character selection and the interface of the editor, and how complex will be the customization. but code wise i dont know how its done, i mean, taking the mesh or model and making those changes, also the hair and that stuff.
about vehicles i can take it as a modular system, les painfull than the character one, for buildings, unless it was on direct terrain, it could be done over a grid.

My editor for a single player 2D game just uses the same code for level loading as the game itself.
The data is then use for 2 things instead of one. A giant text box that is editable with all the world data, and a game window that doesn't have any characters but has debug data shown. And filters for sorting though the data, which is the same code I use in the game for controlling what the player sees.
Don't let this make it sound trivial. Essentially if you were to look at the skill to make/maintain the map editor itself is a lot harder than the real game. The map editor in my case ended up with a bit larger than the real game(though both incomplete), and even larger than that because I'm reusing a lot of code from the actual game(I want to see the same thing). It's also a really bad editor in the sense of workflow(no drag and drop and other nicieties), essentially it was make or literally be able to do nothing.
