You are in a city. You can chose anything from being a crackhead to a doctor. It's just one city with allies, buildings, and districts. There is a red light district. It's gritty and for mature audiences only. PVP free loot. Who would play?

Im not sure if it would attract a large audiance. HOwever, I wager you can put something like that together in second Life
